FIX: Never allow custom emoji to be marked secure (#8965)
* Because custom emoji count as post "uploads" we were
marking them as secure when updating the secure status for post uploads.
* We were also giving them an access control post id, which meant
broken image previews from 403 errors in the admin custom emoji list.
* We now check if an upload is used as a custom emoji and do not
assign the access control post + never mark as secure.

FEATURE: Improving bookmarks part 2 -- Topic Bookmarking (#8954)
### UI Changes

If `SiteSetting.enable_bookmarks_with_reminders` is enabled:

* Clicking "Bookmark" on a topic will create a new Bookmark record instead of a post + user action
* Clicking "Clear Bookmarks" on a topic will delete all the new Bookmark records on a topic
* The topic bookmark buttons control the post bookmark flags correctly and vice-versa
Disabled selecting the "reminder type" for bookmarks in the UI because the backend functionality is not done yet (of sending users notifications etc.)

### Other Changes

* Added delete bookmark route (but no UI yet)
* Added a rake task to sync the old PostAction bookmarks to the new Bookmark table, which can be run as many times as we want for a site (it will not create duplicates).

If our reply tree somehow ends up with cycles or other odd
structures, we only want to consider a reply once, at the first
level in the tree that it appears.

Robin Ward f83362b05b FIX: Don't return post replies from other topics
It seems in some situations replies have been moved to other topics but
the `PostReply` table has not been updated. I will try and fix this in a
follow up PR, but for now this fix ensures that every time we ask a post
for its replies that we restrict it to the same topic.

FIX: Resolve pull hotlinked image and broken link issues for secure media URLs (#8777)
When pull_hotlinked_images tried to run on posts with secure media (which had already been downloaded from external sources) we were getting a 404 when trying to download the image because the secure endpoint doesn't allow anon downloads.

Also, we were getting into an infinite loop of pull_hotlinked_images because the job didn't consider the secure media URLs as "downloaded" already so it kept trying to download them over and over.

In this PR I have also refactored secure-media-upload URL checks and mutations into single source of truth in Upload, adding a SECURE_MEDIA_ROUTE constant to check URLs against too.

FEATURE: Update upload security status on post move, topic conversion, category change (#8731)
Add TopicUploadSecurityManager to handle post moves. When a post moves around or a topic changes between categories and public/private message status the uploads connected to posts in the topic need to have their secure status updated, depending on the security context the topic now lives in.

FEATURE: Secure media allowing duplicated uploads with category-level privacy and post-based access rules (#8664)
### General Changes and Duplication

* We now consider a post `with_secure_media?` if it is in a read-restricted category.
* When uploading we now set an upload's secure status straight away.
* When uploading if `SiteSetting.secure_media` is enabled, we do not check to see if the upload already exists using the `sha1` digest of the upload. The `sha1` column of the upload is filled with a `SecureRandom.hex(20)` value which is the same length as `Upload::SHA1_LENGTH`. The `original_sha1` column is filled with the _real_ sha1 digest of the file. 
* Whether an upload `should_be_secure?` is now determined by whether the `access_control_post` is `with_secure_media?` (if there is no access control post then we leave the secure status as is).
* When serializing the upload, we now cook the URL if the upload is secure. This is so it shows up correctly in the composer preview, because we set secure status on upload.

### Viewing Secure Media

* The secure-media-upload URL will take the post that the upload is attached to into account via `Guardian.can_see?` for access permissions
* If there is no `access_control_post` then we just deliver the media. This should be a rare occurrance and shouldn't cause issues as the `access_control_post` is set when `link_post_uploads` is called via `CookedPostProcessor`

### Removed

We no longer do any of these because we do not reuse uploads by sha1 if secure media is enabled.

* We no longer have a way to prevent cross-posting of a secure upload from a private context to a public context.
* We no longer have to set `secure: false` for uploads when uploading for a theme component.

FIX: Replace deprecated URI.encode, URI.escape, URI.unescape and URI.unencode (#8528)
The following methods have long been deprecated in ruby due to flaws in their implementation per http://blade.nagaokaut.ac.jp/cgi-bin/vframe.rb/ruby/ruby-core/29293?29179-31097:

URI.escape
URI.unescape
URI.encode
URI.unencode
escape/encode are just aliases for one another. This PR uses the Addressable gem to replace these methods with its own encode, unencode, and encode_component methods where appropriate.

I have put all references to Addressable::URI here into the UrlHelper to keep them corralled in one place to make changes to this implementation easier.

Addressable is now also an explicit gem dependency.

In login_required sites, secure uploads should be reusable in any topic, PM or not.

Penar Musaraj 102909edb3 FEATURE: Add support for secure media (#7888)
This PR introduces a new secure media setting. When enabled, it prevent unathorized access to media uploads (files of type image, video and audio). When the `login_required` setting is enabled, then all media uploads will be protected from unauthorized (anonymous) access. When `login_required`is disabled, only media in private messages will be protected from unauthorized access. 

A few notes: 

- the `prevent_anons_from_downloading_files` setting no longer applies to audio and video uploads
- the `secure_media` setting can only be enabled if S3 uploads are already enabled and configured
- upload records have a new column, `secure`, which is a boolean `true/false` of the upload's secure status
- when creating a public post with an upload that has already been uploaded and is marked as secure, the post creator will raise an error
- when enabling or disabling the setting on a site with existing uploads, the rake task `uploads:ensure_correct_acl` should be used to update all uploads' secure status and their ACL on S3

Doing .pluck(:column).first is a very common pattern in Discourse and in
most cases, a limit cause isn't being added. Instead of adding a limit
clause to all these callsites, this commit adds two new methods to
ActiveRecord::Relation:

pluck_first, equivalent to limit(1).pluck(*columns).first

and pluck_first! which, like other finder methods, raises an exception
when no record is found

Krzysztof Kotlarek 427d54b2b0 DEV: Upgrading Discourse to Zeitwerk (#8098)
Zeitwerk simplifies working with dependencies in dev and makes it easier reloading class chains. 

We no longer need to use Rails "require_dependency" anywhere and instead can just use standard 
Ruby patterns to require files.

This is a far reaching change and we expect some followups here.

Robin Ward 31e100530f FEATURE: Flag count in post menu
This change shows a notification number besides the flag icon in the
post menu if there is reviewable content associated with the post.
Additionally, if there is pending stuff to review, the icon has a red
background.

We have also removed the list of links below a post with the flag
status. A reviewer is meant to click the number beside the flag icon to
view the flags. As a consequence of losing those links, we've removed
the ability to undo or ignore flags below a post.

Sam Saffron f8eddd40ad PERF: remove avg_time calculations and regular jobs from posts and topics
After careful analysis of large data-sets it became apparent that avg_time
had no impact whatsoever on "best of" topic scoring. Calculating avg_time
was a very costly operation especially on large databases.

We have some longer term plans of introducing other weighting that is read
time based into our scoring for "best of" and "top" topics, but in the
interim to stop a large amount of work that is not achieving any value we
are removing the jobs.

Column removal will follow once we decide on a new replacement metric.

This commit fixes the follow quality issue with `PostSearchData#raw_data`:

1. URLs are being tokenized and links with similar href and characters
are being duplicated in the raw data.

2. Ligthbox being included in search pollutes the
`PostSearchData#raw_data` unncessarily.

From 28 March 2018 to 28 March 2019, searches for the term `image` on
`meta.discourse.org` had a click through rate of 2.1%. Non-lightboxed images are not included in indexing for search yet we were indexing content within a lightbox. Also, search for terms like `image` was affected we were using `Pasted image` as the filename for
uploads that were pasted.

In terms of indexing performance, we now have to parse the given HTML
through nokogiri twice. However performance is not a huge worry here since a string length of 194170 takes only 30ms
to scrub plus the indexing takes place in a background job.

Sam 384135845b FEATURE: introduce ultra_low priority queue
This commit introduces an ultra low priority queue for post rebakes. This
way rebakes can never interfere with regular sidekiq processing for cases
where we perform a large scale rebake.

Additionally it allows Post.rebake_old to be run with rate_limiter: false
to avoid triggering the limiter when rebaking. This is handy for cases
where you want to just force the full rebake and not wait for it to trickle

Sam e08a3f719c FEATURE: push post rebake regular task to low priority queue
This allows us to run regular rebakes without starving the normal queue.

It additionally adds the ability to specify queue with `Jobs.enqueue` so
we can specifically queue a job with lower priority using the `queue` arg.

Sam 70269c7c97 FEATURE: tighter limits on per cluster post rebakes
We have the periodical job that regularly will rebake old posts. This is
used to trickle in update to cooked markdown. The problem is that each rebake
can issue multiple background jobs (post process and pull hotlinked images)

Previously we had no per-cluster limit so cluster running 100s of sites could
flood the sidekiq queue with rebake related jobs.

New system introduces a hard limit of 300 rebakes per 15 minutes across a
cluster to ensure the sidekiq job is not dominated by this.

We also reduced `rebake_old_posts_count` to 80, which is a safer default.

FIX: Remove `plugin.enabled?` checks at initialization time (#6166)
Checking `plugin.enabled?` while initializing plugins causes issues in two ways:
  - An application restart is required for changes to take effect. A load-balanced multi-server environment could behave very weirdly if containers restart at different times.
  - In a multisite environment, it takes the `enabled?` setting from the default site. Changes on that site affect all other sites in the cluster.

Instead, `plugin.enabled?` should be checked at runtime, in the context of a request. This commit removes `plugin.enabled?` from many `instance.rb` methods.

I have added a working `plugin.enabled?` implementation for methods that actually affect security/functionality:

* FEATURE: Create hidden posts for received spam emails

Spamchecker usually have 3 results: HAM, SPAM and PROBABLY_SPAM
SPAM gets usually directly rejected and needs no further handling.
HAM is good message and usually gets passed unmodified.
PROBABLY_SPAM gets an additional header to allow further processing.
This change addes processing capabilities for such headers and marks
new posts created as hidden when received via email.

Sam 5f64fd0a21 DEV: remove exec_sql and replace with mini_sql
Introduce new patterns for direct sql that are safe and fast.

MiniSql is not prone to memory bloat that can happen with direct PG usage.
It also has an extremely fast materializer and very a convenient API

- DB.exec(sql, *params) => runs sql returns row count
- DB.query(sql, *params) => runs sql returns usable objects (not a hash)
- DB.query_hash(sql, *params) => runs sql returns an array of hashes
- DB.query_single(sql, *params) => runs sql and returns a flat one dimensional array
- DB.build(sql) => returns a sql builder

Sam 89ad2b5900 DEV: Rails 5.2 upgrade and global gem upgrade
This updates tests to use latest rails 5 practice
and updates ALL dependencies that could be updated

Performance testing shows that performance has not regressed
if anything it is marginally faster now.
