The maximum number of fields per index is limited to 1000 by default by the
`index.mapping.total_fields.limit` setting to prevent accidental mapping
explosions due to too many fields. Currently all metadata fields also count
towards this limit, which can lead to some confusion when using lower limits.
It is not obvious for users that they cannot actually add as many fields as
are specified by the limit in this case.
This change takes the number of metadata fields out of the field count that we
check against the field limit. It also adds tests that check that we can add
fields up to the specified limit, but throw an exception for any additional field added.

Add an index level setting `index.mapping.nested_objects.limit` to control
the number of nested json objects that can be in a single document
across all fields. Defaults to 10000.
Throw an error if the number of created nested documents exceed this
limit during the parsing of a document.

* Remove the _all metadata field
This change removes the `_all` metadata field. This field is deprecated in 6
and cannot be activated for indices created in 6 so it can be safely removed in
the next major version (e.g. 7).
This adds the `index.mapping.single_type` setting, which enforces that indices
have at most one type when it is true. The default value is true for 6.0+ indices
and false for old indices.
Relates #15613
Since `_all` is now deprecated and cannot be set for new indices, we should also
disallow any field that has the `include_in_all` parameter set.

This change disables the _all meta field by default.
Now that we have the "all-fields" method of query execution, we can save both
indexing time and disk space by disabling it.
_all can no longer be configured for indices created after 6.0.

Adds infrastructure so `gradle :docs:check` will extract tests from
snippets in the documentation and execute the tests. This is included
in `gradle check` so it should happen on CI and during a normal build.
By default each `// AUTOSENSE` snippet creates a unique REST test. These
tests are executed in a random order and the cluster is wiped between
each one. If multiple snippets chain together into a test you can annotate
all snippets after the first with `// TEST[continued]` to have the
generated tests for both snippets joined.
Snippets marked as `// TESTRESPONSE` are checked against the response
of the last action.

This catches *some* bugs in the docs during build which is a good start.
Using files that must be specified on each node is an anti-pattern
from the API based goal of ES. This change removes the ability
to specify the default mapping with a file on each node.

When set to false a new strict mode of parsing is employed which
a) does not permit numbers to be passed as JSON strings in quotes
b) rejects numbers with fractions that are passed to integer, short or long fields.
