fix(bundles): rename UMD bundles 

BREAKING CHANGE:

UMD bundles were renamed:
* `angular2.umd.js` -> `angular2-all.umd.js`
* `angular2-testing.umd.js` -> `angular2-all-testing.umd.js`

Closes #5898
