Minor adjustments, in particular, gets rid of custom styling in favor
of using plain markdown for pipe example. This fixes the layout for
some displays.
Followup to #2990
Tweak to `.gitignore` so that only the `tsconfig.json` files in
subdirectories are ignored, not the one directly in this `_examples`
folder.
Also, add a package.json script definition for `http-server` for more
convenient access (w/o invoking tsc as is forced in the script
definition given in `_boilerplate/package.json`.

Rob Wormald recognized that we had no plunker for a simple component test. Inspired improved learning path for testing including:
* Add plunkers for both inline and external template versions of the simplest `BannerComponent`
* Added the banner-specs for that purpose and also a quickstart-specs in the setup folder
* Adjusted prose in testing and setup-systemjs-anatomy to call these out
* Moved testing of external template spec earlier in the guide because it is likely to be needed right away.
* Add comments on the optional "testing" folder and corrects var names to match

Only the page TOC should be at heading level 1. All other headings
should start at level 2. This commit also removes duplicate headings,
in continuation of #2796.
(This fix is necessary to support an auto-TOC feature.)

No need to dual boot `AppComponent` and `ProvidersComponent` only to
ensure that we could demo optional injection; instead just inject null
for the `@Optional` constructor parameter type.
No prose is affected by these changes.

- Dart non-api doc pages w/ links to API entries will now have those
links translated from a TS API reference URL to a Dart API reference
URL.
- Created and ran a link checker to verify that all such links to API
entries refer to Dart.
- Fixed links to pages that have moved:
- JS & Dart: `i18n` is under now `cookbook` not `guide`
- Dart: testing page is now directly under guide.

* plunker builder removes files and scripts not needed in plunkers (e.g., systemjs.config.js)
* no longer adds placeholder README.md (deemed a distraction)
* tag is now "Angular" instead of "Angular2"
* new plnkr.json "includeSystemConfig" option adds systemjs.config.js back; needed for testing plnkrs.
* incidentally changes comment "Polyfill(s)" to "Polyfills" in all index.html
Bug in Jasmine v.2.5.x. Stick with v.2.4.1 (as karma-jasmine does) until the following issue is resolved:
