* Introduced fab!, a helper that creates database state for a group

It's almost identical to let_it_be, except:

 1. It creates a new object for each test by default,
 2. You can disable it using PREFABRICATION=0

Sam Saffron 9be70a22cd DEV: introduce new API to look up dynamic site setting
This removes all uses of both `send` and `public_send` from consumers of
SiteSetting and instead introduces a `get` helper for dynamic lookup

This leads to much cleaner and safer code long term as we are always explicit
to test that a site setting is really there before sending an arbitrary
string to the class

It also removes a couple of risky stubs from the auth provider test

Guo Xiang Tan 24347ace10 FIX: Properly associate user_profiles background urls via upload id.
`Upload#url` is more likely and can change from time to time. When it
does changes, we don't want to have to look through multiple tables to
ensure that the URLs are all up to date. Instead, we simply associate
uploads properly to `UserProfile` so that it does not have to replicate
the URLs in the table.

Sam Saffron 4ea21fa2d0 DEV: use #frozen_string_literal: true on all spec
This change both speeds up specs (less strings to allocate) and helps catch
cases where methods in Discourse are mutating inputs.

Overall we will be migrating everything to use #frozen_string_literal: true
it will take a while, but this is the first and safest move in this direction

Robin Ward ba6d4b2a8d FIX: Better handling for toggling `must_approve_users`
If you turn it on now, default all users to approved since they were
previously. Also support approving a user that doesn't have a reviewable
record (it will be created first.)

This also includes a refactor to move class method calls to
`DiscourseEvent` into an initializer. Otherwise the load order of
classes makes a difference in the test environment and some settings
might be triggered and others not, randomly.

Sam e6b3310577 FIX: never redirect back to `/sso` it will cause a loop
If for any reason our return url is set to `/sso` bypass using it
for login redirect

Sam aa044623bd FIX: do not create superflous sessions when logged on
In some SSO implementations we may want to issue SSO pipelines for
already logged on users

In these cases do not re-log-in a user if they are clearly logged on

Maja Komel 27e732a58d FEATURE: allow multiple secrets for Discourse SSO provider
This splits off the logic between SSO keys used incoming vs outgoing, it allows to far better restrict who is allowed to log in using a site.

This allows for better auditing of the SSO provider feature

Sam 89ad2b5900 DEV: Rails 5.2 upgrade and global gem upgrade
This updates tests to use latest rails 5 practice
and updates ALL dependencies that could be updated

Performance testing shows that performance has not regressed
if anything it is marginally faster now.

Guo Xiang Tan 7fc8a36529 DEV: Take 2 Queue jobs in tests by default.
On my machine this cuts the time taken to run our test suite
from ~11mins to ~9mins.

Sam 0134e41286 FEATURE: detect when client thinks user is logged on but is not
This cleans up an error condition where UI thinks a user is logged on
but the user is not. If this happens user will be prompted to refresh.

implemented review items.

Blocking previous codes - valid 2-factor auth tokens can only be authenticated once/30 seconds.
I played with updating the “last used” any time the token was attempted but that seemed to be overkill, and frustrating as to why a token would fail.
Translatable texts.
Move second factor logic to a helper class.
Move second factor specific controller endpoints to its own controller.
Move serialization logic for 2-factor details in admin user views.
Add a login ember component for de-duplication
Fix up code formatting
Change verbiage of google authenticator

add controller tests:
second factor controller tests
change email tests
change password tests
admin login tests

add qunit tests - password reset, preferences

fix: check for 2factor on change email controller
fix: email controller - only show second factor errors on attempt
fix: check against 'true' to enable second factor.

Add modal for explaining what 2fa with links to Google Authenticator/FreeOTP

add two factor to email signin link

rate limit if second factor token present

add rate limiter test for second factor attempts
