Rewrite of ManagedSelector to handle connection creation as an ExecutionStrategy task.
...
Now the creation of a connection, and the Connection.onOpen() call
happen as a Runnable that is run by the ExecutionStrategy.
This allows onOpen() to block or otherwise perform tasks that are not
suitable to be run by a selector thread, since the ExecutionStrategy
will guarantee that another thread will take over the selector duties.

Improved handling of the stream close state.
...
Now the stream close state is updated when the frame has been
successfully written, and when it is received.
The stream is closed in case of failures.
Just after the stream close state update, if the stream is closed
then it is removed from the session.

459963 - Failure writing content of a committed request leaks connections.

Fixed by making the logic in HttpSender fail and complete the response
in case of request failure, without taking in consideration whether
the request was committed or aborted.

Simplified HTTP/2 parser and its listener.
...
It is not possible to perform asynchronous processing of the content
of DATA frames, because otherwise the parser has to stop, stalling
all other streams.
Parser.Listener methods were returning boolean in a vestigial attempt
to handle asynchronous data processing, and have now been converted to
return void.

Adding TooFastClientTest for websocket server
...
+ test performs a client upgrade + 2 websocket frames all at once.
not waiting for the upgrade response before sending those frames.
+ currently set to @Ignore until we can address how to copy this extra
buffer information from the Http side to the WebSocket side.
